Which theme are you using? Maybe the theme doesn't support user pictures. If it doesn't then you'd have to manually enter the php in the theme's template files. Or use another theme.
If the options are greyed out, you should visit administer -> user settings
scroll to bottom of page and enable the user pictures first, then go back to administer -> themes -> config and set the proper checkboxes

Go to administer > Site Building > Themes > Configure.
Then check the box beside "User pictures in posts" and/or "User pictures in comments".
