I have searched all over and cannot find: Where is the book outline or what should I do to create one. I use Apache & Linux server and a Linux workstation. 4.5.2 [the folders are dated:15-Jan-2005].

Somebody mentioned that the outline tab appeared after reinstall on Linux. Tried it and still no go.

Any suggestion or pointer will be appreciated
