Empty user name after registration

When using this module sometimes an empty username is stored in the database when creating the user.
I have spent a time to figure out on what circumstances it happens - without luck while now... Any ideas?
On registration form I'm using some profile fiels (such as name, surname etc.), I'm using "Reg with pic" module - allow user to upload the images when registering. Also
