Is it possible to kill the Clean URL requirement? I'd like to use your module, but because of server limitations I cannot enable Clean URLs on my multisite subdomains.

This requirement definitely could be removed, as I would hate to punish users who don't have Clean URL access. However, due to time constraints, I am going to be unable to get to coding this for probably another couple of weeks. If you'd like to change it, the requirement code is within pictures.module, but I'm not sure how that would break creating the filenames. Also, it would get rid of one of the benefits of Apache reading the file directly off the server, instead of having to pass it through drupal, although, there may be some way to get drupal to pass the 404 error code which would be generated, through this module, so that we could force it to generate the cached image, and then send it as the result (similar to how it generates it now).

I've convinced my host to provide the config that allows Clean URLs. This is no longer necessary for me, but you may still want to offer the option to bypass Clean URLs for other users.

Sorry for the delay on replying.
Since I needed to upgrade the module to incorporate other things which were required, I've created a module called Media Manager. It doesn't have the Clean URLs requirement, but provides an extended functionality.

This module is being "retired". Please refer to the above mentioned module, since it will perform all the features of the this module, and I'll be maintaining http://drupal.org/project/mmedia instead.
