Adding fields as awell as axis' for specific content types

Is it possible to add the option for users to submit fields as well as criteria axis' as a kind of survey instead of review. For example, if I want to have users submit a review of a company I want them to rate it on various criteria (taken care of by the axis option) but I also want to ask each reviewer to input their salary and information like how long they worked there for (among a number of other fields). I think this is possible but not sure just how to go about it. Any help would be great.
