Official release from tag: 5.x-1.0

First stable release of CAPTCHA pack, now that CAPTCHA version 5.x-3.0 (with the new CAPTCHA API) is released.

This release contains a new challenge: the ASCII art CAPTCHA

Other minor bugs and issues solved since 5.x-1.0-rc2:

minor code style fixes and removal of obsolete statements
removed 'version = "$Name$"' as suggested by http://drupal.org/node/101009
#174974: CAPTCHA type for validation phase is now the same as in the original generated form
#178008: solves bug with disabling a challenge module after enabling it as random CAPTCHA candidate
