Would be nice if this module could insert birthdays automatically into the calendar module (optional).

Calendar is based on views. You should be able to add the birthday profile field in views! I'm not really sure about D5

Bumping to 7.x. Won't happen (unless someone writes a straight forward patch) for D5 and might be backported to D6.
