2) If you have existing hierarchies, be sure to set up the existing
database by going to lineage/reset. You only need to do this once,
though there is no harm in doing it again.

Btw lineage/reset does not work and I cannot identify anything similar in the sourcecode for lineage

This is no longer an issue because lineage actually runs the update query when it's enabled. To run it again you can disable the module and uninstall it, or use the devel module to 'reinstall lineage'.
