Add created_at and updated_at dates to db table to allow for expiring unanswered relationship requests

We would like to be able to expire relationship requests that haven't been responded to within a configurable timeframe. To do this, we need to keep track of when relationship requests were created. While we were at it, we thought it might be useful to also keep track of the last time a relationship was updated. Attached is a patch file for your review.
