Menu names Missing! (in Admin)

in http://jbsoulscan.com/admin/build/menu if u wanna edit the menu, you dont see the menu's name whic u already create.

Underneath

Menus are a collection of links (menu items) used to navigate a website. The list(s) below display the currently available menus along with their menu items. Select an operation from the list to manage each menu or menu item.

You see:

(Menu Name is missing, other themes display them, like Mind & Body)
> instead i have a button add comment ?!?!?

same thing happen to me here: www.uphone.com.ph I am using D6.5

What I observed was these:

1. I use to access the site from different computers which I was using previously.
2. I did some administration tweak in the user permission module to one of the computer and then saved it.
3. when I log out, and then trying to log in again as admin or any registered user: viola! that's the page "access denied".
4. When I use another computer that I used previously when all things are running smoothly and I type NOT the domain uphone.com.ph but the public static IP address of 121.97.78.188 then I can access the site and all the menus for the admin esp. the ADMINISTER menu.
5. When I cleared the private data: cookies, cache, etc of my browser then again viola!: "access denied"
6. Even I used the public static IP of my domain name still I can't access the page as admin.
7. If i request a new password as admin, there's no delivery in my registered mailbox but if I request a new password as ordinary and authenticated user I can access all the pages and menus and the access denied message not appearing.
8. However, when I log out the said ordinary and authenticated user can't log in again and NEED to request again another automated password.

Is there anyone here tried to fix this one.

Any help will back my mind in the normal state.
