To reproduce:
Change the label of a field that has field permissions enabled.
After changing this label, you should see that the field disappears in both the node create/edit form as well as the CCK manage fields form.
Then go to admin/user/access and save the settings without changing any of them.
The field you re-labeled should now appear again.
