I'm getting duplicate node references when creating a node. See the attachment - I just type that in and you can see what comes up in the node form. My fallback is autocomplete if that is an issue.

I've gotten this bug too.
In the beginning, I was using select list and set to six items. I then changed to reference url. The reference remembered that I could have a maximum of six items and duplicated the reference to match. I had to go back, change to select list, and either choose 1 or unlimited items. For pre-existing references, I had to fix the reference while in select list mode, not url widget mode.
