"Some default forms are already provided in the form list," I see no any list. And if I try to add for example user_register, save - it does nothing. Return to settings and list is empty as it was. Other settings are saved. http://screencast.com/t/jhYgajU6JN

Hi,

is that on a fresh install or is it possible that you had an old/outdated version of the CAPTCHA module installed?

have you already tried to disable-uninstall-and-reinstall the module?

I had this same issue on a site that was upgraded from D6 to D7. It turns out I had forgotten to run update.php after enabling on D7. Running update.php did the trick.
