commit 79c222549eafa538965d2d79d56641d4ad791659 causes a bug when i press "add more". As you can see from the "after" screenshot, a third blank item is created.

Also, the previous values that were saved in this field collection become orphaned.

core is 7.16, entity is latest dev and php is 5.4

As far as I know this should be working in the latest versions. I notice from the screenshots that you seem to be using field_collection_table for your form so make sure you have the latest version of that too.
