After adding a 4th level of conditional options this module doesn't seem to work, anything after 3 levels is always displayed despite the conditional options. Is this a bug or just a limitation of the module?

It is not a known bug. If you make very simple form with an export and upload it to this issue, I or someone else can take a look at it. Instructions here.
