The client migrated their site to a new server. Unfortunately we did not have much of a hand in that process, and came into the project post-move.
Example of broken web form:

If you visit a web form, like in the above example, none of the fields are actually displayed on the page. When I inspect the source, only the hidden inputs are being rendered on the the page.
If I go to Edit -> Form Components, I can see all of the components added to the form. If I click on "edit" for a form compenent, the edit screen appears "empty". (I can see the rest of the page, and the view, edit, results, etc tabs are still there.
If I try to create a new form and add a new component, upon hitting "add" for that new component I am taken to a similar "empty" edit screen as described above.
What I've tried:
I have double checked that I have all permissions for my administrator user for this module. Have also created an entirely new role and user and assigned permissions to and still the same.
Have double checked that the template files include the necessary php to output the form.
Have double checked that it is not CSS hiding any elements.

vernond commentedSeems that either $form['submitted'] is getting clobbered somewhere along the line, or perhaps the components don't know that they're supposed to be assigned to node 92 (check the `nid` column in the webform_component table of the site database).
[edit] Could you provide a little more info regarding the "empty" component edit page. Is it empty as in "the page is blank with nothing for me to fill in", or are the fields that should contain component info all blank?
