Um, say I wanted to install taxonomy_access and was running Drupal CVS - do I still need to use the supplied taxonomy.module patch, or would that only work on the 4.5.2 version?

i think it should be > 4.5 .
i think it should be > 4.5 . Without patch i dont think it will work, though i havent used under 4.5.2 but quick look at the file implies that it is required.
you may also want to check the bug reports on the module and validate against your result.
