Add new platforms for contact

Although there are no real social media platforms, but the option to link to a contact form or e-mail address is certainly useful.

Add new form fields to

• add a new node address (like the RSS field)
• add a e-mail-adress

In the default iconset, the mail icon can used for both platforms.
