Ampersand (&) is not replaced, nor removed. I tested this in 5.x-2.x-dev and 5.x-2.0-beta4.
Instead it's replaced with amp.

Which kind of object is this (user, node, term) and what is your pattern for the object?

I imagine that you are using a [title] when you should use [title-raw] is that right?
