Term edit form always loads default values

On taxonomy terms, I can edit meta tag title and description. After saving, my changes work as expected.

However, when I edit the term again, the default values load (instead of my edited values), so saving causes the title and description to revert to the default values.

I am using panels 7.x-3.3, ctools 7.x-1.2 with the following patches
