I wasn't able to log-in on my site while it was in maintenance mode because the math CAPTCHA wouldn't accept the right numeric answer. When I removed the clientside_validation module from the server, I was able to again access my site (WHEW!).

I really have no other info to add but if you have specific questions please ask. This error completely freaked me out, completely, like thoughts of 9 months of development down the drain kind of "start over" thoughts of being locked out. *sigh*

I did no specific per-form changes or set any per-form settings, all defaults at this point.

Oh, and I have no idea if it's the same "bad math" with this module enabled and not in maintenance mode (online).

And if this helps, I did have image CAPTCHA selected but it won't work in maintenance mode and defaults to math CAPTCHA, dunno if that defaulting caused the validation to hold onto the wrong correct answer.
