simple image upload form based on gallery name

Hi,

Please give me a simple code idea to upload image and its description based on gallery name.

Gallery names are listed.
User can select any gallery and add the new image on that gallery.

Help me ..

Give simple code format.

I suggest rather than trying to code it yourself you look into an existing module - http://drupal.org/project/node_gallery and it's coordinating modules should do what you need.

it aids to know which version of drupal is in use.
