Search text box on the top right of page

Hi,

On Firefox and Chrome the search textbox it's at the bottom of the search selector by site/users.

If we search something this message appear

Notice: Undefined variable: original_keywords in custom_search_submit() (line 384 of /var/www/beta1/profiles/commons/modules/contrib/custom_search/custom_search.module).
Search is temporarily unavailable. If the problem persists, please contact the site administrator.
