The checkboxes are not being displayed while using the administration theme calle "shiny"
Attached capture screen is more explicit.
Thing are normal after reverting to default admin theme "Seven".

I don't think that is a problem form the taxonomy_ manager module, I think that it's an issue for the shiny theme issue queue.

Finally, I have upgraded to Dev version of 2013-Jan-22 and I can get access now to the check-boxes.
