Auto-complete form returns a HTTP 500 error

I have tried re-installing the module, and uninstalling some other modules such as Javascript tools I thought may have conflicted - but same result

Modules Installed:

Core
Imagechache
Javascript tools
Cutom Pagers
Smackdown
Thickbox
SWF Tools
Views Voting API
Yahoo YUI

Somebody else had this problem as well, but I was unable to recreate it. I'll recreate your setup and see what I can find.
