On the main faq page, e.g. "www.mysite.com/faq" you should only see the "Home" link in the breadcrumbs. If using categories and you're viewing a faq category page, e.g. "www.mysite.com/faq/123" then the breadcrumbs should be "Home >> Frequently Asked Questions". If viewing a faq node in the "My Account" category, then the breadcrumb should be "Home >> Frequently Asked Questions >> My Account".
This works in the garland theme. Some themes disable breadcrumbs or treat them differently, so if it also affects other pages on your site, then the issue is probably with your theme.
Unfortunately, setting a path alias doesn't place the pages in a site hierarchy, which requires menus. But never mind, I'll work around it by putting faq in primary navigation.
