I have installed the LM_Paypal module and the premium module. I know I can have users who already have accounts create paypal subscriptions to obtain specific user roles.
Has anyone set it up so that you have to do the paypal subscription BEFORE you can create a user account? The idea is that only members will be able to create accounts, and to become a member you have to pay via paypal.

I think the easist way to pull this off is to allow users to create an account (authenticated). Don't give many if any permissions to this account. They have users pay to upgrade from authenticated to a user role with more access.
Doing it this way allows you to offer a minimal amount of content (if using something like taxonomy access) allowing them to wet their whistle and then upgrade their account.
MoneyScripts
Hi I have installed a module called moneyscripts. You have to pay $50 for it but it's well worth it because you don't need 3 modules and a convoluted method. http://www.moneyscripts.net/drupal-membership-suite

except that it's not open source, hasn't been tested by simpletest in public, hasn't gone through any public security screenings.
This is not to say that the module isn't any good. I don't know. I've never used it, nor do I know how the developer does what he/she does because its closed.
