Document issue management best practices

Let's create a book page (or at least update the project description) with links to best practices around issue management. Here are a few suggestions:

1. Each task should be owned by someone and no issues should be unassigned;
2. When mentioning other issues or linking to them, use [#000] notation;
3. There's no need to include "GLADCamp" in the name — all issues are already about GLADCamp;
4. Update your email notification settings at https://drupal.org/project/issues/subscribe-mail/gladcamp  
5. Etc., etc.
