Hello,
When I rotate image without change title, I get this:
image not find
I need to change the title and save again to get the new image display.
or
Go to the image (item) node and save it.
URL OK
I think it is because of the image name is changed into xxxxx_r[1|2|3|4].xxx ...

I need more information about how to reproduce your error. On my site, I am able to rotate images without changing the title, using both the radio buttons and popup. It updates the image live, and on save, it saves the new orientation.
