The custom output does not use ubercart's theme functions uc_price, etc. This results in, eg. unformatted prices on the checkout and review pages.

Patch Seems to work fine for me.

Also looking at the theming, "Discount" and "Subtotal" aren't lined up and "Discount" isn't bolded. (I'm sure I can fix through CSS for the time being)

Same here; patch works, still minor formatting discrepancy from std Ubercart pane.
