I modified the module so that you can select the role you want from a drop down list on the user registration form. The admin can choose which method (i.e. select role before registration form, or select role on registration form) is preferred.

While I've tested this patch to make sure that it works with the new feature, I haven't tested that the old feature still works. (Though I think it should.)

I kinda placed this functionality into the new module. It now has an admin interface to allow the public end user to select a single role or multiple rolls. It presents these as a group of radio or checkboxes, but not a select box.
