GoDaddy Linux hosting, MySql database.

When adding a classified ad with the 'add one' link the taxonomy selection doesn't display in 'Create Classified Ad Form' (see screenshot below). So after saving the ad it apparently is not tagged with 'For Sale' or 'Wanted' and doesn't show up in the main list, both terms show 0 ads. If you click on either term the ad doesn't show up in listing of either term.
This occurs when using my custom theme or a core theme.
It also occurs when logged in as registered user or user1.
The terms 'For Sale' and 'Wanted' have been added to taxonomy and show up as category terms on classified ad page but not on the 'Create Classified Ad' page.
Curiously there is both a Classified Ad Categories taxonomy AND a Classified Ad Catagory taxonomy (see screenshot below). I assume one (which one?) is left over from a previous version.

This looks like like a site upgraded from 5.x (bluemarine theme ?), or a previous version of the module (6.x-1.x / 6.x-2.x) without full uninstallation : there is no upgrade path to 6.x-3.x / 7.x-3.x from older versions.

Can you reproduce the problem on a fresh installation, not one evolved from an earlier configuration ?

you were correct - it was a previous version of the module 6.x-2.x without full uninstallation. I failed to read the Readme.txt file and loaded in 6.x-3.0 with out following the instructions in Readme.txt.
For the benefit of anyone else who has the same problem: I deleted 6.x-3.0, reverted back to 6.x-2.0-alpha6 and followed instructions in the Readme.txt file.
However after Installing 6.x-3.0 like it says at the end of the Readme.txt file it gave me an error "vocabulary missing. Please re-install the module." But that alone fails to fix it, you have completely uninstall the 6.x-3.0 module with the uninstall option on modules page, then re-install the module, then it will work.

Since there is no upgrade path to 6.x-3.x it would be nice if you included a stronger warning on the project page telling people to be sure and read the Readme.txt file. A lot of us have a bad habit not reading those files (oops).
