CleanURL Test Failing. What is it testing?

I've been beating my head against this particular wall for the past week, posted all over the place and still can not get the CleanURL test to pass. I've tried the easy "paste in mysite.com/admin/config/search/clean-urls" trick with no success.

So, I was wondering if anyone can tell me just WHAT it is looking for? I'm going to dig in but I'd love some direction as to what will make that test happy.

The test could be failing for many different reasons but most likely it's a server config issue. That being said, I simply Googled drupal clean url test and this was the first link that came up:
