Properly handling extensions

Although the validation code properly checks for phone extensions, the submission code doesn't handle the 4th matched expression (the extension). This patch tacks on the extension, if provided. A further patch to the module would be nice if users would like to change the "extension sign" from the 'x' that is currently hard-coded.
