not autofill of license setting on forums - mysql error

Issue Summary

* Creative Commons: Please submit the title for your content.
* Creative Commons: Please submit the description for your content.
* Creative Commons: Please submit the creator for your content.
* Creative Commons: Please submit the rights for your content.
* Creative Commons: Please submit the source for your content.

when i hit submit, just a plain txt page says this:

when i press the preview button instead, a well formed page appears with the site headers and blocks saying:
