I just tried using the domain_conf module without realizing that there was an extra step (require_once in settings.php) required in order to get the module to work.

It would be nice if you could display some kind of warning/error message when a user goes to the Domain site settings page to let them know if they they to modify settings.php

Good idea. Done with a new hook_domaininstall() that runs on the Domain Access configuration page.
