STATEMENT OF WORK
Basic Requirement:
The Wizard will allow users to complete a PDF online (can be in Non-PDF format).
The Form is two pages
Some areas (such as name of the user, title of their WORK, and their address) will be automatically completed. - Our users will be submitting written poems to our website. Our data base will contain:
Their name
Their address
The name of their poem
The poem itself.
Perhaps some other typical information about the user.
These items will have to be taken and automatically placed in the proper fields. For example, there is a spot on the form that asks for the name of the person who wrote the form. Also, there is spot that asks for the mailing address of the person who wrote the form. Where we have data that matches something being asked on the form, we want to prepopulate that entry but also give the user a chance to review the entry and make changes
All areas automatically completed are reviewed by user and user may change the enter. When they use the Wizard, and when we autopopulate an entry, a window should Pop up, like this:

Areas that are not populated automatically must be filled in by user. Please check the form in detail to see the fields which needs to be filled in. The form that is to be completed can be found here:

NOTE, there is a two page explanation that precedes the form. This two page explanation document is NOT shown to the client as a part of the form.
HELP for user –
The process will take the user through each step of the completion of the form. Each step is numbered on the form, so that’s how many steps there are. Each step will have a ? button, if they click it, they will see text appear in a window that explains that particular step.
HELP comes in two formats
1. Help text that we will provide for each entry, in plain English.
2. The official help text (the first two pages mentioned above) that will
be copied directly, but only that help for the relevant section will appear when requested by user.
SO: For example, when prompted for the NAME OF THE WORK
A blank will appear that client may fill in (or is automatically completed already if the title exists in our database but must be subject to review and editable from user)
HELP, if requested, offers (a), our version of an explanation of what is needed for the section being completed and (b) the official government explanation for the section. The user can pick one or both to read, alternatively.
The completed form will be printed by us. The PDF file should be saved in a specific folder on the server with ongoing numbers in a folder named by date for archiving purposes. The folder for the day must be created automatically and all generated PDF will be saved in this folder. This should happen for every day 24 hours bases. The reason why is, we have to print all documents of the day on our local printer and send it physically to the government agency. If you have an idea how to make this procss better and more automated let me know.
The last part of the form requires a signature. We will print our attorney's signature at that area (you have to make that work as well), rather than having him hand sign the signature. According, the “agent' box will always be checked. A comprehensive step by step expectation for each of the areas will be provided.
However, at this point, the explanation for the job can be simplified as follows:
A. One two page document will be completed by way of a wizard, suitable for printing.
B. Some of the entries for the document will be automatically completed, drawing from a data base. Name, Address (which may be listed more than once on the document), title of the work and so forth.
C. Some of the entries will not be automatically completed.
D. A COPY OF THE WORK WILL ALSO BE PRINTED IN DUPLICATE, AT THE SAME TIME AS THE FORM ITSELF AND MUST BE PRINTED AT THE SAME TIME SO THAT THE WORK (IN DUPLICATE) AND THE FORM CAN BE EASILY HANDLED FOR MAILING TO THE GOVERNMENT. THE FORM CANNOT BE SUBMITTED ELECTRONICALLY (intelligent printing concept).
The government requires the following to be filed:
The Form.
Two identical copies of the printed work (poem).
A Check for the filing fee
You just have to work out the printing of the form
This has to be a independent Drupal module which can be integrated in to any Drupal CMS. Pleas check here http://drupal.org/node/508 It needs to be compatible to other Drupal modules a s such as:

Workflow:
The user opens up the form.
The Wizard takes the user through each step of the form. Note that the steps are numbered on the form itself. We will match that numbering.
So, as an example
1. Step One: Title of the Work
Since we already will have the title of the work in our database, that title should be pre populated on the form. But the user should be able to review what is being put there and have a chance to change the title if he wants.
There will also be the above mentioned ? function that explains this step. I will provide the text.
2. Step Two: This requires the following to be put on the form in the correct location:
Name and Address of the user
Nationality (USA or whatever)
Phone fax and email (in the correct locations.)
All of this information should be in the Drupal CMS user profile and as such, available for pre population. Note that each element should be able to be changed by user manualy as well.
And so forth.
Please review the form and get a feeling for what is going in the form. That would help a lot.
I will also prepare a written introduction to the form for the user, which should be read by the user before starting the actual process.
Summary:
Basicaly this are the major points:
1 There is a link which starts the wizard
2 The introduction opens
3 The form starts. Some fields are pre populated but manualy editable by user and some fields are empty to be filled in by user.
4 When chosing the work to be copyrighted there should be a drop down menu showing all of users work in our database where he can choose from which one she/he wants to copyright.
5 Finalize the process by redirecting the user to the payment gatway.

Deliverables:
1) Complete and fully-functional working Drupal module in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows:
a) Only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) The buyer intends to distribute: A module installation package that will install the software in ready-to-run condition on the platform(s) specified in this request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
