Reduce the admin interface to one page.

Since the modules to include are now being chosen within the modules selection page, there is no longer a need to have the 'modules' section of the activity admin interface where modules to provide activity for is chosen.

Attached is how I would like to redesign the current interface. Certain 'ops' and 'types' (like content types for nodeapi) will still have to be checked off, but the text boxes will not appear until checked.

As a first iteration make a version that has all the form elements there with or without the checkboxes. We've got to keep the form functional in a non-JS environment.

Otherwise looks good!

The current 5.4 and 6.1 branches have an improved admin interface with
a certain amount of javascript reveal/hide but it isn't fully working and
needs some help.
