I am having the same issue.
There's a comment box already on a node. You click on reply to reply to an existing comment, another box (the reply box) comes up - exactly like the existing making it two. This is very obvious if you have only a few comments on a page. It's best to hide the existing form when a user clicks reply and show it back when the user refreshes or cancels from reply.
