What am I missing? Cannot see how to add text overlay. Have tried both 7.x-1.1 and 7.x-1-dev.
Installed and enabled both ImageCache Actions and System Stream Wrapper.

You need to enable the module "Image Text Effects". After having done so, you should see the effect 'Text' in the effect drop-down list. If it does not appear try to clear all caches. If it still does not appear feel free to reopen this issue.
