Hi,
just testing this module i note that hook_init doesn't work without clean url enabled. Furthermore i did some performance improvements.

I haven't touched the '_url_access_uuid' but it's included in the patch (maybe a dos/linux prob?)

Thanks for catching this. I've committed the changes to the -dev branch, and seems to fix the issues on my test site. Let me know if it works for you and I'll tag 1.1.
