Add to README

Took me awhile to actually get it working. Wasn't that there was anything wrong with it but just that everyone's problem seems to be the same and different at the same time.

Basically I went through the support post and it didn't help me.

Until I looked at the module settings again.

The README should state that under Visibility that node/* needs to be removed so it shows up as node/* implies that it is disabled on everything. As I didn't even think of that and spent around 30 minutes looking at my theme source and playing around with input settings.

Yea, works fine now.
