Swap "path" and "title" fields on "add items" page

Issue Summary

Steps to reproduce: open, for example, the block creation page and try to configure or add a new block.

Now look at the add items page: "path" and "title" fields are reversed comparing to the other configuration or creation interfaces. Just as suggested at [1], swap them.
