Several Issues on New Install

Just installed og_galleries with latest version of image. Having this problems:

Thumbnail images don't display, only titles in boxes (see attachment)

Galleries Duplicated (see attachment)

"Main" default subcategory does not show in "Galleries" box on image node submit form. "Main" is the default gallery subcategory created. I created 2 additional subcategories under my Group. What I'm saying is when I create an image node, only the 2 additional subcategories appear, and NOT "Main".

On image node save and/or edit:
