Custom Checkout Pane for Ubercart

Seeking an Ubercart, Drupal, and PostgreSQL experienced programmer for a small job to create a custom checkout pane on the Ubercart checkout screen to include three checkbox fields for fields within the profile (profile_[fieldname]). The three checkboxes are for Privacy Policy, Terms of Use, and Code of Conduct and a Thickbox link should be included with each to the actual files. Optionally, I would like to only include this checkout pane if the user is not already a member and not currently logged in.

I am using Drupal 5.5 with PostgreSQL.
