A book with leaf nodes and folders

Hi,

I'm trying to use the book module and let users have a book in which they will create hierarchical structures of folders and pages inside.

How can I:
- Give each user one book with a predefined name when he registers?
- Create "leaf" node type that a user can not add child pages to.
- Create a node type that can't be attached to a book at all.
- Give a granular permission to per node type for users to be able to add as child nodes to a book.
