help needed

Installed quicktags, having issues on three seperate installs on two different servers.

Had to apply patch to get it to work : http://drupal.org/node/198326

On create content pages, I get the normal box to enter main text, but the title for it is now "B:", and there is a letter B under the box as well. The quicktag buttons show up above the box......but most of the time do nothing when you click them, and if they do anything its random at best(inserts tags random places in the text).

There is also a new box under "Input Format". This is a smaller version of the main text entering box and is titled "L:", with a small L under the box. Works the same as above box.

Not sure if im doing something wrong or....?
