Database not created - Sliced Book with drupal 5.5

error message as follows:
user warning: Table 'drupalneurodyn.slicedbook_navigation' doesn't exist query: SELECT attribute FROM slicedbook_navigation WHERE nid = 9 in /var/www/htdocs/drupal/includes/database.mysql.inc on line 172.
Database is not created on install of module. Running update.php does not work either
Have tried using the dev version as well with the same results. Attempts to create database table as per query in slicedbook_install does not work either. We had sliced book working previously with earlier builds of drupal 5.0
