To add tilte and the description to attachment it is not possible and "administer/configuration/modules/attachment" path not works.

Can you be a little more precise ?
I got a fresh Drupal 4.6.1 installed, with attachement 4.6 and it works.
Also the administer/configuration/modules/attachment path, is not very clear in INSTALL file, it's just to say to activate attachement module in order to have it working. See http://drupal.org/node/23327 and http://drupal.org/node/21104

I have removed the confusing test in the INSTALL file.
