When I use the now standard [book-raw] pattern the URLs keep [book-raw] in them, instead of the token being replaced. Other raw patterns, such as [title-raw] etc are working OK.

Yes, same issue. I left as active instead of duplicate since the issue you link to is now a token issue (and so not visible in the pathauto issues list). Feel free to change that.

Well, I generally dislike havingmultiple active issues for the same topic because it can split the conversation into multiple areas.

I've attached a patch there - please test it and let me know if it works. If so, then I will apply it.
